Transitional support
Moving from the family home or leaving a residential college can be daunting for many younger people reaching adulthood, but at Mosaic Community Care our aim is to make that move as smooth a transition as possible. Our team is available to offer advice on a range of issues, from housing provision, day time activities, employment and care right the way through to individual budgets and benefits.
Wide range of transitional support services
Whether you are a young person who uses services, a family/carer or commissioner, we can allay any fears you may have, and ensure you receive timely and relevant information that will help you choose the right package that meets your requirements. If you are looking for us to provide supported living, whether in a family home or as a tenant of a housing provider or private landlord, we have a range of services available, tailored to every person’s needs. Whether you are looking for a few hours support a week or 24 hour care, our expert teams at Mosaic are on hand to provide assistance.
Transition planning
By undertaking transition planning at an early stage, even as young as 14 years old, and involving each person and their circle of support at every step of the way, we can help alleviate fears and anxieties, and make the transition from children’s care services to adult care a much easier one.

Warrington Disability Partnership had this to say about Mosaic and the winning award. "The nominators of this winner are professional foster carer's and look after children with complex needs. Over the last 18 months they have been involved with 2 children who's development is greatly impacted on by their social and play/leisure time activities. Both children's complex needs could easily socially isolate them.
